The Nursing Home Transition (NHT) program was created to help re-balance the long-term care system in Pennsylvania so that people in need of long-term services could receive them in the setting of their choice, including their homes.

Gathering Information
In order to assist transitioning individuals in applying for housing and/or benefits, you may need to assist them in obtaining documents verifying certain important information. Below is a list of commonly required documentation. Included in the list below, where available, are hyperlinks to sites where copies of documentation can be requested.
