***NOTE: Any NF that submitted a portion of their assessment fees before 5/8/2021 should ensure they are completing an ACH or wire transfer only for the remaining balance owed for assessment year 2020-21 (Q69 through Q72).
*Resident Day Reports not submitted by the established reporting deadline will result in penalties assessed as described in the PA NF Assessment and Quarterly Resident Day Reporting Form End User Manual.
**Assessment Payments not paid by the established deadline will result in interest assessed beginning the day after the Assessment payment is due, unless the facility has an approved payment plan in accordance with the PA NF Assessment and Quarterly Resident Day Reporting Form End User Manual. Quarterly Assessment payments must be a wire transfer or ACH Credit will be treated as unpaid and assessed interest; See the PA NF Assessment and Quarterly Resident Day Reporting Form End User Manual.
