Request a replacement card | If you require a replacement card for the 2021-2022 school year, Summer 2022 or Summer 2023, you can request a new card through the P-EBT

Activate my P-EBT card or change my PIN
| You can activate your card and change your PIN by calling the number on the back of the card. For a P-EBT card, you should enter the SSN using 000-00 as the first five numbers and then the last four digits of your zip code.

Check my P-EBT balance and transaction history | You can check your balance and transaction history, including deposits and spending transactions, online through
www.connectebt.com or the ConnectEBT mobile app.
You can also call the number on the back of the card for an automated list of options to check the balance of the card and hear the last ten transactions on the card.
