For Community Providers
Please find the attached resource toolkit (Post Acute and Long Term Care COVID-19 Toolkit zip file) developed by Pennsylvania’s Department of Health to assist you with COVID 19. There are helpful documents to assist with preparedness assessment, tracking logs, notification letters to families and more. Approaches and documents may need modification as applicable for your community homes and service provision models.
Community providers should also review guidance from the Centers for Disease Control on Recommended precautions for household members, intimate partners, and caregivers in a nonhealthcare setting.
