Legal/Family Guardian

A court appointed representative who has the authority to act on behalf of a Waiver/Program participant.

Participant

Children from birth to age five who have special needs due to developmental delays and disabilities who are enrolled in an Office of Child Development and Early Learning (OCDEL) Waiver (e.g. Infants, Toddlers, and Families (ITF) Waiver) and receiving Early Intervention supports and services.

Family Member

Spouses, parents, or other related persons who support children through their transition into a new program, classroom, or school. They will be required to sign a HIPAA compliant authorization form.
