A recent audit uncovered recipient date of death discrepancies in the Department’s eligibility records. This audit identified recipients who were deceased prior to the end date of the billed service. The Department performed a claims recovery adjustment to recoup monies paid in error. These adjustments are identified on this remittance advice by ICN range 5218086346582 through 5218086348003.

It is your responsibility to resubmit a correctly completed claim if you are due payment for services provided prior to the date of death.
