The NHT program was created to help re-balance the long-term care system in PA so that people in need of long-term services could receive them in the setting of their choice, including in their homes. Please note that the following information applies to the fee-for service NHT program administered by the Office of Long-Term Living and does not apply to Nursing Home Transition programs or services provided through Community HealthChoices (CHC), which are an administrative function of the CHC Managed Care Organization (MCOs). Individuals who are enrolled in CHC and wish to return to the community from a nursing facility should be directed to their MCO for assistance.
